• Healthcare Provider Access: If you use a referral code from your healthcare provider, we share your treatment adherence data, engagement metrics, progress tracking, and clinical assessments with that provider to support your care. This sharing is limited to information necessary for your treatment.‍
• Service Providers: We work with HIPAA-compliant vendors who help us operate our services (such as cloud storage and data security providers). These partners are contractually prohibited from using your data for their own purposes.
• De-identified Data: We may use aggregated, anonymous data that cannot identify you for clinical research, quality improvement, and developing better behavioral health treatments.
• Legal Requirements: We may disclose information when required by law, court order, or to:
  • Comply with public health reporting requirements
  • Report suspected abuse or neglect as mandated by law
  • Prevent serious and imminent threats to health or safety
  • Respond to health oversight activities
• Insurance Processing: With your consent, we may share information with insurance companies solely for billing and reimbursement purposes.

Account and/or Data Deletion Process for Verity Health app

1. Request Submission: To initiate the account and/or data deletion process, please contact us at info@verityhealth.io and indicate your request to delete your account and/or specific types of data.
2. Verification: Upon receiving your request, we may verify your identity to ensure the security of your account. This may involve asking for additional information to confirm your identity.
3. Confirmation: You will receive a confirmation email within 2 business days indicating that your request has been received and is being processed.
4. Deletion Timeline: After verification, your account and/or associated data will be deleted permanently. This process is irreversible, and your information cannot be recovered once deleted.

Information We Keep when you Delete your Account

Upon requesting your account deletion, we will keep none of your personal data. Your entire account and all associated data will be deleted forever, and unable to be restored at a later date.

SMS Communications

By providing your phone number through Verity or during enrollment with a participating provider, you consent to receive SMS messages related to your care, including appointment reminders, confirmations, and other service-related notifications.

Message frequency varies based on your care and appointment schedule.

You can opt out of SMS communications at any time by replying STOP. For assistance, reply HELP or contact support at info@verityhealth.io.

Verity does not sell or share your phone number with third parties for marketing purposes.

User Responsibilities

Before initiating the deletion process, users should back up any data they wish to retain, as all information will be permanently deleted and cannot be recovered.

Exceptions

In compliance with applicable laws, we may retain certain information as necessary to fulfill legal obligations, resolve disputes, or enforce our agreements. Any retained information will be subject to our data retention policies and will not be used for any other purpose.

Data Security

We take reasonable measures to protect your personal information from loss, theft, misuse, and unauthorized access, disclosure, alteration, and destruction.

We prioritize data safety by adhering to HIPAA compliance standards and employing encryption to protect all associated data.

In the event of a data breach that may compromise your personal information, we will notify you using the primary email address associated with your account.
